Your visit is your first step towards living the life that you want and deserve.  Whether you are coming in in need of help managing chronic pain, relief from headaches, or desire an improvement in your sports performance, you have found the right place.

At your first visit you will spend about 45 minutes at the clinic.  We will take you through a functional based assessment and figure out not only what specific injury you have but also the cause and underlying problem. Finding the cause for your injury means we will work towards getting you out of pain as well as working on the underlying problem making it less likely to have this injury return.

We understand that filling out paperwork can be monotonous. However,  it is necessary and thoroughly filling out your new patient paperwork is one of the most important things you can do to help us at Identity Chiropractic. This paperwork allows the doctor to identify any additional symptoms that could be complicating your condition while allowing us to properly diagnose, plan and implement a treatment protocol.

Please make sure you fill the required paperwork sent in an email before coming to your first appointment or show up about 15 minutes early so you leave yourself plenty of time!


Several health insurance companies in Kansas and Missouri currently cover chiropractic care and related therapies.  However, some companies have varying levels of coverage.  It is best to check with your health insurance company first in order to see if you have eligibility and coverage for our services. 

Identity Chiropractic, at this time, DOES NOT submit (or bill) claims to insurance companies.  But if your insurance company will reimburse  "OUT OF NETWORK PROVIDERS" (please check with your insurance company first), we will provide you the necessary legal documentation in order for you to obtain reimbursement from your health insurance company.  We have chosen to do this in order to provide our patients the care that they need without the limitations of insurance. Your care should be dictated by what you need! We will always do our best to serve your needs.  Please let us know this information as it will be assumed that you will not be submitting a claim to your insurance company otherwise.
